Patient Information

Appointments: Patients must have a physician’s referral in order to make an appointment with Olympia Neurology. If you must cancel your appointment, please notify us at least 24 hours in advance. Call 360.413.8550 and select Option Two for appointment scheduling and cancellations.

Billing: If you have any questions about billing, please contact our Billing Department at 360-413-8551.

Forms:

Please download, print, complete, and sign both the Insurance Information Form and the Medical History Questionnaire. Bring these (Medical History, Intake Sheet) with you to your first appointment.

Insurance:

Olympia Neurology is contracted with most insurance companies.  If you have any questions regarding your coverage, please contact your insurance company.

  • Co-Payments – If your insurance company requires you to pay a co-payment, you will be required to provide payment before you are seen by one of our physicians. We accept cash, checks, Mastercard, and Visa. There is an additional $15 charge if a bill is generated for your co-payment.
  • Insurance Cards – You will be required to show your current insurance card or medical coupon at all office visits. Please contact your insurance company to verify that you have coverage for your visit and for any testing that is scheduled.
  • Motor Vehicle Accidents – It is our policy to treat motor vehicle accident patients as cash patients. We do not bill auto insurance. However, we will generate a claim form you can forward to the insurance company so that you can be reimbursed upon request. Full payment will be required on the day of your visit.
  • Referrals – If your insurance requires a referral, it is your responsibility to make certain that it is received in writing by our office prior to your appointment date. If you are unsure if a referral is required, please contact your insurance company. Please note that some insurance carriers can take up to five business days to respond.

Medical Questions or Concerns: Call 360.413.8550 and select Option Three for medical questions or concerns. You will be triaged by a medical assistant. If you are connected to voicemail, please leave your name, phone number, date of birth, date and time of your call, and a brief message. Calls will be returned according to urgency.

Medication Refills: To request a medication refill, please call your pharmacy.  Kindly allow at least two business days to refill your medication.  If you have questions about the medication our physicians have prescribed, please call  360.413.8550 and select Option Three. 

Office Hours: We are open from 9:00 a.m. until 5:00 p.m., Monday through Friday.
